Personalised Care for Vulnerable Transitions
When vulnerable individuals transition out of rehabs, women’s shelters, prisons, or into supported housing, they often face uncertainty and a lack of direction. Organisations tasked with supporting them are frequently under-resourced, leaving these individuals unsure of how to access mental health support, housing, employment opportunities, or life skills training.
To address this, I designed and prototyped an AI-driven solution that automates the creation of personalised care plans. This system collects key data—such as location, employment history, interests, mental health background, and aspirations—through GDPR-compliant Microsoft Forms. Once submitted, the data is sent to a GDPR compliant trained AI model that has been provided with a comprehensive data set that has all of the resources available in the local area, and generates bespoke care plans tailored to the individual’s unique needs.
The resulting document includes detailed guidance on accessing mental health support, pathways to volunteering or employment, housing options, benefits advice (whether securing them or transitioning away), life skills training, and finding meaning and purpose. It also highlights tools and opportunities they might never have considered, such as free courses, community groups, and other resources aligned with their interests. The care plan is generated directly into the organisation’s files, ready to be shared and discussed with the client, freeing staff to focus on one-on-one sessions and meaningful interactions.
My prototype was designed to create a unique “life builder” care plan, but any AI solutions I provide for your organisation are built specifically for you. It can be tailored to fit the specific goals and expertise of any organisation, addressing their clients’ needs in a way that enhances, rather than replaces, existing care plans. From Portsmouth to any other area, the AI model can be trained on local data, and customised to do particular kinds of analysis and generate particular kinds of responses, ensuring every care plan is actionable, localised, and transformative.
